House of Hope’s Outpatient Program provides flexible, person-centered treatment for individuals seeking help with substance use disorder, mental health challenges, or both.

Individual Counseling

Our outpatient team offers one-on-one counseling for adults navigating substance use disorder or mental health concerns. Sessions are tailored to each person’s needs and may focus on:

  • Building coping strategies
  • Relapse prevention skill building
  • Managing stress, anxiety, or depression
  • Strengthening motivation for change
  • Developing goals for long-term recovery

Clients work directly with licensed counselors who bring experience, compassion, and evidence-based approaches to every session.

In addition to one-on-one counseling, clients have access to case management services that support the practical side of recovery. Case managers help connect individuals to community resources, coordinate care, and address needs such as housing, employment, or transportation.

Foundations of Hope

Foundations of Hope is a structured, one-on-one educational program designed for individuals who meet some criteria for Substance Use Disorder (SUD) and would benefit from early intervention. This program focuses on prevention by increasing understanding, building skills, and supporting healthier decision-making before more severe symptoms develop.

Participants receive education and guidance in:

  • Overview of addiction neuroscience
  • Risk factors for developing SUD
  • Practical prevention strategies
  • Substance-specific modules on alcohol, cannabis, stimulants, opioids, and nicotine
  • Stages of Change and how motivation shifts over time
  • Introduction to levels of care and how treatment pathways are determined
  • SMART goal-setting to support value-aligned behavior change

Foundations of Hope offers a practical, supportive starting point for individuals who may be at risk and benefit from early awareness, skills, and prevention-focused intervention.

Assessments

Our outpatient program offers comprehensive assessments for individuals seeking treatment recommendations, meeting court or probation requirements, or completing referrals for outside agencies. Assessments are conducted by trained professionals who provide clear guidance, appropriate referrals, and supportive next steps.
